Nursing and rehabilitation centers across the country have adapted to meet the diverse needs of todays patients and residents. When people leave the hospital, they often need continued care in order to recover completely. That is where we come in, bridging the gap between hospital and home.

Short-Term Rehabilitation or Transitional Care and Rehabilitation

As patients have become more medically and clinically complex, we have focused on services to meet their changing needs. Our Transitional Care and Rehabilitation Centers are shaping the future of our nursing and rehabilitation centers. These centers focus on patients who require short-term rehabilitative care. Generally they are recuperating from joint surgery or other procedures and need an intensive, supervised rehabilitation regimen. Rehabilitation services include physical, occupational and speech-language therapies.

Outcomes Excellence

We demonstrate our dedication to quality care and performance improvement through the outcomes data that we measure and report. Concordia Care monitors re-hospitalization rates, length of stay, discharge disposition, functional outcome measures and customer satisfaction. We utilize this data to improve our quality and build post-acute care relationships that help reduce avoidable re-hospitalizations, establish appropriate patient goals and plans of care for successful return to home.

Dementia and Alzheimers Care

While we are advancing our rehabilitative services, Concordia understands a great percentage of our residents have some sort of dementia, including Alzheimers disease. Many of our nursing and rehabilitation centers specialize in the unique challenges facing these residents and offer a supportive and safe environment for the resident, and peace of mind for their families.
